About the Coordinating Center
The University of Michigan serves as the Coordinating Center and is responsible for assisting the participating teams as needed to help them achieve their care transitions goals.  The Coordinating Center organizes the collaborative-wide meetings on a quarterly basis.   The Coordinating Center has 2 partners: St. Joseph Mercy Hospital and Society of Hospital Medicine.   See Partners for additional information.

Coordinating Center Leads
Christopher Kim, MD, MBA
Chris Kim

Dr. Kim is an Assistant Professor both in Medicine and Pediatrics at the University of Michigan. His clinical practice is in hospital medicine at the University Hospital.  In addition, Dr. Kim serves as the Assistant Chief of Staff in the Office of Clinical Affairs and Associate Medical Director for the Faculty Group Practice where he works on several quality improvement and patient safety initiatives for the health system.  He received his Medical Degree from the University of Chicago Pritzker School of Medicine and MBA from the University of Chicago Graduate School of Business.

Julie Wietzke

Julie Wietzke is a Project Senior Manager in the Division of General Medicine, Department of Internal Medicine at the University of Michigan.  Prior to her position in the HMS Coordinating Center, she has been employed by the University of Michigan for over 10 years in various roles associated with the management and administration of research.  She received her Health Management and Policy degree from the University of Michigan School of Public Health and her Library Science degree, specializing in health sciences information, from the University of Maryland College of Information Studies.
